Discover a Better Life with Wellbeing Exeter's Self-Referral Programme Wellbeing Exeter's Community Connector Self-Referral Pathway

People living in Exeter and Cranbrook are being offered an exciting new opportunity to improve their wellbeing and connect with local communities.

Wellbeing Exeter is launching its Community Connector Self-Referral Pathway programme and is looking for adults who might like to get involved.

Joining the program will enable individuals to access personalised support and resources to help them live their best lives.

The self-referral programme allows participants to gain access to one-on-one support from a Community Connector who will work with them to explore the opportunities available in Exeter and Cranbrook.

To be eligible for the program, participants must be over 18, live in Exeter or Cranbrook, and not already be registered with Wellbeing Exeter or Wellbeing Cranbrook.

Through the self-referral programme, participants will have the chance to discover new opportunities on how to live and feel better, talk to someone and be listened to, meet new people and get to know the community better, learn new skills, and get connected to what matters to them. They will also receive information about available resources and opportunities to help them achieve their goals.

Wellbeing Exeter is a partnership organisation funded by Exeter City Council, Devon County Council, NHS Charities Together, NHS Primary Care Networks, and Sport England.
